Gummy Smile BOTOX® Explained: How Muscle Relaxation Changes Lip Elevation
Updated: Sep 29

INJECTABLES
Showing gum when smiling is not inherently abnormal. Some people simply have a higher smile line. When a patient wants to reduce the amount of gum that shows, the first question is why the smile appears gummy, because muscle activity is only one possible contributor.

Why a gummy smile can occur
Upper-lip elevation, tooth proportions, gum anatomy, lip length, and the vertical position of the upper jaw can all influence gingival display. More than one factor may be present at the same time.
A neuromodulator can only affect the muscular component.
How neuromodulator treatment works
Small amounts of botulinum toxin may be placed into selected elevator muscles around the upper lip. Reducing some muscle activity can limit how far the lip rises during a smile.
The goal is moderation rather than eliminating normal smile movement.
Why placement must be conservative
The muscles around the mouth contribute to expression, speech, and lip position. Too much weakening can produce an asymmetric or heavy smile.
Small dose changes can have a visible effect in this area, which is why incremental treatment is important.
What treatment cannot change
Neuromodulators cannot shorten gum tissue, change tooth size, or reposition the upper jaw. If those factors are dominant, dental, periodontal, orthodontic, or surgical evaluation may be more appropriate.
A muscle-based treatment should not be used to compensate for a structural cause.
Results are temporary
As with other neuromodulator treatments, the effect gradually wears off as nerve signaling and muscle activity recover. The duration varies between individuals.
Maintenance should be based on actual return of movement rather than an arbitrary calendar.
Why facial balance matters
Gingival display should be assessed in the context of lip length, smile pattern, teeth, and facial proportion. Reducing upper-lip elevation is useful only when muscle activity is meaningfully contributing to the concern.
When BOTOX® may help a gummy smile
Neuromodulators can soften a muscle-driven gummy smile in selected patients, but the treatment only makes sense when excessive lip elevation is a meaningful part of the cause.
